The State of Arizona requires all hospital employees to submit a Level 1 fingerprint clearance card along with the application. If you already have a valid Level 1 clearance card you do not need to get reprinted. Arizona requires all applicants to go through a criminal background check to ensure the safety of the patients and other staff in the facility.
