In the state of Arizona, you are required to provide an FD-258 fingerprint card with your application if you are applying for a liquor license. If you are part of an individual applicant, part of a Corporation, LLC, or Partnership and have 10% or more control, you will need to provide prints to the state of Arizona with an application. Once you have it on file, the state will run a background check every 2 years.
Your fingerprints will be used to check the criminal history records of the FBI. If you have a criminal history record, the officials making a determination of your suitability for the liquor license must provide you the opportunity to complete or challenge the accuracy of the information in the record. You should be afforded a reasonable amount of time to correct or complete the record (or decline to do so) before officials deny you the Liquor License based on information in the criminal history record. This disclosure is from AZ Dept. of Liquor License and Control.
Fingerprinting Services Arizona is a bona fide fingerprinting service that uses only FBI-approved fingerprint cards. Come by anytime Monday through Friday to take care of your fingerprinting for AZ Dept. of liquor licenses and control. No appointment necessary.
