The state of Arizona requires all new mortgage broker applicants to submit FD-258 fingerprint cards (no other card will be accepted) in order to initiate a background check. A full criminal background check will be done through the FBI. Some companies will allow you to use existing prints so if you already have a FD-258 fingerprint card you won't need to get a new one.
